Some time ago trustmaster make multicat plugin
I think that this plugin need be included into core, but with some changes: multicat use no extra field but defailt cat field And user choose the category in witch he want to see his page by clicking checkboxes or lines in select multiple="multiple" and i think that it is not right: when admin of page category add page it must automatic validated. Agreed on autovalidation. Disagreed on multi-cat, because most of sites don't need it and the way you have described it will be a significant slow-down.

Multicat very useful for most modern sites. Maybe developers create a finished multicat plugin? It would be the most optimum decision
