I highly recommend the question based captcha. Since my site primarely is for a special language audience, I can use localiced questions which the bots are not able to answer. Absolutely no spam until now.
Grafical captchas are no longer the best solution, not only because of accessibility issues. I'm visually impaired and use Webvisum to read them out, and it's likely that even the spammers have such tools. Steffen signature://doesnt.exist

That's a definite overdose: most users will leave your site forever rather than trying to figure out what
in welcher Sprache stelle ich dir diese Frrage? (Klingonisch ist es nicht):is. I would assume that ANY captcha is a 99% protection for a low-to-medium ranking site which 99% of all Cotonti sites are. If a spammer is trying to place ads at your site, "ban by email" is good for nothing, since switching to a different public mailserver is a one-minute job. As regards "top-10" megasites, an overcomplicated captcha might inevitably cause loss of audience in comments (don't make me think!). Such websites require "manual" moderation anyway, so once again this is not the case to play quiz with your unauthenticated visitors. SED.by - создание сайтов, разработка плагинов и тем для Котонти

Sorry, but people who say "Don't make me think!" should be banned from the whole web. Because many of them are responsible for the huge success of spammers, fishing websites and other malicious things.
Every kind of captcha has it's disadvantages. And it's also a question of audience. But people who are able to decode strange grafics (many of them have their difficoulties, not just blind people) should be able to answer simple questions. (The above is asking in which language the question is by the way) And if people would use their brain a bit more when browsing the web, the need of over-compicated captchas would be much lower.
