I highly recommend the question based captcha. Since my site primarely is for a special language audience, I can use localiced questions which the bots are not able to answer. Absolutely no spam until now.
Grafical captchas are no longer the best solution, not only because of accessibility issues. I'm visually impaired and use Webvisum to read them out, and it's likely that even the spammers have such tools.
