Well I've always used Neocrome but I've taken an interest to this after reading about it a while back on their site. And I'm wondering if I use the current 0.0.5 release, and than update to 0.0.6 when that gets released, would I have to completely reinstall from scratch if I customize my website design? Or would the update just update tags and such?

Just run the sql and freshen cms files.

Skin files structure would be different (0.0.5-ish should work though). I'd recommend starting with 0.0.6.
