You need to add the icon into \images\pfs
Iconname: extenstion.gif For you dtwt.gif and ptf.gif ... can we help you ...

What's the URL they are requesting from? Just check the source. You're getting 404 errors so just check which directory you forgot to upload to.

oh, im so silly, @pieter you were correct and @Lombi thanks you where the reasion i checked thinking i put it in the pfs, but i put it in the system/img/pfs
