Hooks=index.tags
Tags=index.tpl:{EZSLIDER_SLIDESHOW}

Replace that with:

Hooks=header.tags
Tags=header.tpl:{EZSLIDER_SLIDESHOW}

deinstall the plugin, and install it with the new hooks and tags \
